Registration for TK

 When registering your child, who has not previously been enrolled, the following documents are required: 
 
1. To prove age: Education Code section 48002 permits school districts to accept the following: 
  • Certified copy of birth record
  • Statement by the local registrar or county recorder certifying the date of birth
  • Baptism certificate
  • Passport

 

When none of the foregoing is obtainable, the parent/guardian may provide any other appropriate means of proving the age of the child. 
 
A student enrolling in a district school at any other grade level shall present records from his/her previous school district documenting his/her age and current grade level. 
 
(As required by State law, all children entering kindergarten must be five years of age on or before September 1) 
 
2. Immunization Verification: 
All children are required to have their vaccinations fully completed before enrolling in school. Immunization Record to include the following
  • Diphtheria, Tetanus and Pertussis (DTap, DTP, or DT)
  • Polio - Measles, Mumps, Rubella (MMR)
  • Hepatitis B
  • Varicella (Chickenpox)
(A Personal Belief Exemption Form is no longer an option) 
 
3. Proof of Residency: (only one proof is necessary)
  • Property tax payment receipts
  • Rental property contract, lease, or payment receipts
  • Utility service contract, statements, or payment receipts
  • Pay stubs
  • Voter registration
  • Correspondence from a government agency
  • Declaration of residency executed by the parent or legal guardian of the student
 
While a district must accept the listed documents, the district may choose to accept other types, as well. 
 
4. Other required documents needed (Grade Specific):
  • Transcript from previous school (6-12 grade students only)
  • Provide a copy of the withdrawal form from the previous district
  • Oral Health Assessment (Transitional Kindergarten-Kindergarten students only)
  • Physical Exam (1st grade students only)
  • Court order regarding student custody or any other legal documentation concerning the student

Al inscribir a su hijo, que no ha sido matriculado anteriormente, se requieren los siguientes documentos: 

1. Para demostrar la edad: La sección 48002 del Código de Educación permite que los distritos escolares acepten lo siguiente: 
  • Copia certificada del acta de nacimiento
  • Declaración del registrador local o del registrador del condado certificando la fecha de nacimiento
  • Certificado de bautismo
  • Pasaporte
 
Cuando ninguno de los anteriores se puede obtener, el padre / guardián puede proporcionar cualquier otro medio apropiado para probar la edad del niño. 

Un estudiante que se inscribe en una escuela del distrito en cualquier otro nivel de grado deberá presentar los registros de su distrito escolar anterior documentando su edad y el nivel de grado actual. 

(Según lo requerido por la ley estatal, todos los niños que ingresan a kindergarten deben tener cinco años de edad en o antes del 1 de septiembre).
 
2. Verificación de vacunas: 
Se requiere que todos los niños tengan sus vacunas completas antes de matricularse en la escuela. La Cartilla de Vacunación debe incluir lo siguiente
  • Difteria, tétanos y tos ferina (DTap, DTP o DT)
  • Polio - Sarampión, Paperas, Rubéola (MMR)
  • Hepatitis B
  • Varicela
 
(El documento de exención por creencias personales ya no es una opción)
 
 
3. Prueba de residencia: (sólo es necesaria una prueba)
  • Recibos de pago del impuesto sobre bienes inmuebles
  • Contrato de alquiler, contrato de arrendamiento o recibos de pago
  • Contrato, extractos o recibos de pago de servicios públicos
  • Talones de pago
  • Registro de votantes
  • Correspondencia de una agencia gubernamental
  • Declaración de residencia firmada por el padre, la madre o el tutor legal del alumno
 
4. Otros documentos necesarios (específicos del grado):
  • Certificado de notas de la escuela anterior (sólo para estudiantes de 6º a 12º grado)
  • Proporcionar una copia del formulario de retiro del distrito anterior
  • Evaluación de salud dental (sólo estudiantes de kindergarten de transición)
  • Examen físico (sólo estudiantes de 1er grado)
  • Orden judicial relativa a la custodia del alumno o cualquier otra documentación legal relativa al alumno
